Every athlete needs a signed injury & liability waiver on file before their first practice. It takes two minutes — read and sign it right here on the site, and it's filed automatically.

Every athlete needs a waiver signed by a parent or legal guardian. Read it, sign below with your finger or mouse, and it's filed with the club automatically — you're done in two minutes.
Please read carefully. This document affects your legal rights.
1. Voluntary Participation. I am the parent or legal guardian of the athlete named below ("Athlete") and I voluntarily consent to the Athlete's participation in sports programs, practices, conditioning, games, competitions, performances, and related activities (the "Activities") organized by South Coast United Sports Club ("the Club"), including basketball, flag football, cheer, and track & field.
2. Assumption of Risk. I understand that participation in youth sports involves inherent risks that cannot be eliminated regardless of the care taken to avoid injury, including but not limited to: falls, collisions with other participants or objects, sprains, fractures, concussions and other head injuries, heat-related illness, cardiac events, permanent disability, paralysis, and, in rare cases, death. I understand these risks and knowingly and freely assume all such risks, both known and unknown, on behalf of the Athlete and myself.
3. Release and Waiver. To the fullest extent permitted by law, I, on behalf of myself, the Athlete, and our heirs, assigns, and next of kin, hereby release, waive, and discharge the Club and its owners, directors, officers, coaches, volunteers, employees, agents, sponsors, and facility owners (the "Released Parties") from any and all claims, demands, causes of action, or liability of any kind arising out of or related to the Athlete's participation in the Activities, including claims arising from the ordinary negligence of the Released Parties.
4. Indemnification. I agree to indemnify and hold harmless the Released Parties from any claims brought by or on behalf of the Athlete arising out of participation in the Activities, to the fullest extent permitted by law.
5. Medical Treatment Authorization. In the event of an injury or medical emergency, I authorize the Club's staff to obtain emergency medical treatment for the Athlete, including first aid, ambulance transport, and treatment by licensed medical providers. I understand I am responsible for all medical costs, and that the Club does not provide medical, health, or accident insurance for participants.
6. Physical Condition. I certify that the Athlete is in good physical condition and has no medical condition that would prevent safe participation in the Activities, and I agree to inform the Club of any relevant medical conditions, allergies, or medications.
7. Rules and Conduct. The Athlete and I agree to follow all Club rules, coaches' instructions, and codes of conduct. The Club reserves the right to dismiss any participant whose behavior endangers themselves or others.
8. Photo & Media Release (optional). If checked below, I grant the Club permission to use photographs and video of the Athlete taken during Activities for team, promotional, and social media purposes, without compensation.
9. Severability. If any portion of this agreement is held invalid, the remainder shall continue in full force and effect. This agreement is governed by the laws of the state in which the Club operates.
By signing below, I certify that I am the Athlete's parent or legal guardian, that I have read and understood this entire agreement, and that I am signing it freely and voluntarily.
